COLLECTION OF YOUR PERSONAL INFORMATION
This Privacy Policy applies to information we collect: (i) on the Site, and (ii) in email, text and other electronic messages between you and this Site.

This Privacy Policy does not apply to information collected by: (i) us offline or through any other means, including on any other website operated by the Company or any third party, or (ii) any third party, including through any application or content (including advertising) that may link to or be accessible from or through the Site.

MY POLICY WITH MINORS.
The Site is not intended for individuals under the age of 18. No one under age 18 may provide any personal information to or on the Site. We do not knowingly collect personal information from children under 18. If you are under 18, do not use or provide any information on this Site or through any of its features, make any purchases through the Site, or provide any information about yourself to us, including your name, address, telephone number, email address, or any screen name or username you may use. If we learn we have collected or received personal information from a child under 18 without verification of parental consent, we will delete that information. USE OF YOUR PERSONAL INFORMATION AND CONFIDENTIALITY.
If you do choose to provide your personal information, we will not willingly share your information with companies outside our organization, except as described in this Privacy Policy.  You may at times receive communications from us related to products and services that we believe might interest you. While we believe these services may enhance your time spent at the Site, you will at all times have the option and ability to opt out from receiving these communications by specifically choosing to do so via a link which will be provided within emails that we send to you. We may disclose total aggregated user statistics in order to describe my services to potential advertisers, other third parties, affiliate companies, and for other lawful purposes. 
The information we gather from you may be used in several ways, either now or in the future, to gain a better understanding of the Site’s users and their usage pattern as a whole, for site administration and troubleshooting, to process transactions, contest entries and other matters you initiate, to identify preferences in content and advertising, to target editorial, advertising or other content (such as promotions, special offers or other content) we think might be of interest to you. DATA SECURITY
We have implemented measures designed to secure your personal information from accidental loss and from unauthorized access, use, alteration, and disclosure. However, the safety and security of your information also depends on you. Where we have given you (or where you have chosen) a password for access to certain parts of our Site, you are responsible for keeping this password confidential. We ask you not to share your password with anyone.

Unfortunately, the transmission of information via the internet is not completely secure. NOTICE OF PRIVACY PRACTICES FOR CLIENTS OF LET’S TALK WITH ASHLEY PORCELLI, LLC
This notice describes how health information about you may be used, disclosed, and accessed as client of the Company. We recognize that your health information and the care you receive are deeply personal, and protecting your privacy is a priority of ours. We maintain records of the care and services we provide in order to ensure quality treatment and to comply with applicable law. This notice describes the ways in which we may use and disclose health information about you, and your rights to such information. We are committed to ensuring that protected health information (“PHI”) is kept private, in accordance with applicable law. For the purposes of this Section, PHI refers to any individually identifiable health information that is created, received, maintained, or transmitted by Company related to the services provided hereunder. PHI includes, but is not limited to, information such as patient names, addresses, birthdates, medical histories, diagnosis details, treatment plans, and any other information that can be used to identify a person and relates to their physical or mental health.
Company agrees to use and disclose PHI only as necessary to perform the services described herein and in compliance with applicable laws and regulations, including the Health Insurance Portability and Accountability Act (HIPAA). Company will not use or disclose PHI for any other purpose without the express written consent of the individual to whom the PHI pertains, except as required by law. Under federal privacy regulations, health care providers may use or disclose your health information without your written authorization for specific purposes such as treatment, payment, or health care operations. For example, we may share your health information with another licensed health care provider in order to support your diagnosis or treatment. 

In no event will we use or disclose your PHI for marketing purposes, nor will we sell your PHI in the regular course of our business. Subject to certain limitations in the law, we can, however, use and disclose your PHI without your authorization for the following reasons:
When disclosure is required by law;
To report crimes, or suspected child, elder, or dependent adult abuse;
To prevent or reduce a serious threat to health or safety; 
For research purposes, including studying and comparing the mental health of patients who received one form of therapy versus those who received another form of therapy for the same condition; 
To contact you to remind you that you have an appointment with us. 
To explore treatment alternatives, or other health care services or benefits that we offer; and
To a family member, friend, or other person that you indicate is involved in your care or the payment of your health care, should you consent to do so. Please note that consent may be obtained retroactively in emergency situations, in compliance with applicable law. 

We keep “psychotherapy notes” as that term is defined in 45 CFR § 164.501, and any use or disclosure of such notes requires your authorization unless the use or disclosure is: (i) for our use in treating you; (ii) as required by law; or (iii) as required to help avert a serious threat to the health and safety of others. Other than “psychotherapy notes,” you have the right to receive a copy of your medical record and other information that we have about you. We will provide you with such documents within 30 days of receiving your written request, and reserve the right to charge a reasonable fee for doing so.
Company agrees to implement appropriate administrative, physical, and technical safeguards to protect PHI from unauthorized access, use, or disclosure. This includes ensuring that all employees, agents, and subcontractors who may come into contact with PHI are trained on the confidentiality requirements and have signed confidentiality agreements as necessary.

You have the following rights with respect to your PHI:
To ask us not to use or disclose certain PHI for treatment, payment, or health care operations purposes, provided that we may decline such request if we believe it would negatively affect your health care.

To request restrictions on disclosures of your PHI to health plans for payment or health care operations purposes if the PHI pertains solely to a health care item or a health care service that you have paid for out-of-pocket in full.
To request a list of instances in which we have disclosed your PHI for purposes other than treatment, payment, or health care operations, or for which you provided with an authorization. 

Upon termination of your services with the Company, Company will promptly return or destroy all PHI in its possession unless retention of such information is required by law. If PHI cannot be returned or destroyed, Company agrees to extend the protections of this Section until such time as it is lawfully permitted to dispose of the information. In the event of any breach of PHI security, Company will immediately notify you of the breach. Notification will occur in accordance with HIPAA and other applicable data protection laws and regulations. Company will cooperate in all efforts to mitigate the harm caused by the breach.
